Tusks of Mannoroth are a pair of plate shoulder guards once worn by Garrosh Hellscream.
The fangs originally belonged to Mannoroth — a powerful pit lord and high‑ranking demon of the Burning Legion. Grom Hellscream managed to kill Mannoroth, freeing the orcish clan from demonic domination. Later, Mannoroth’s skull was mounted in Orgrimmar as a monument to Grom’s heroic deed.
The fangs themselves became a symbol of that victory and eventually came into the possession of Garrosh Hellscream.

How to obtain Tusks of Mannoroth
These epic shoulder guards have a 0.2 % drop chance from Garrosh Hellscream in the Siege of Orgrimmar raid.
To access this raid, travel to the Jade Forest via the portal room. Then mount a flying mount and head to the Vale of Eternal Blossoms.
Near the entrance to Mogu’shan Palace, you’ll see a white pillar. The raid entrance is located at its base.

To obtain the shoulder guards, you must defeat Garrosh using one of the following classes:
Paladin
Warrior
Death Knight
Once acquired, you can use their appearance with any class that can wear plate armor.
Notes
Besides the standard drop method, the shoulder guards were available for purchase during the Mists of Pandaria: Remix event. However, there is currently no information about whether this event will return.
The shoulder guards can occasionally be found for sale on the Black Market Auction House, though they have only appeared there a few times.
Mannonoth’s Fangs can be transmogrified either on the right shoulder (as Garrosh wore them) or on both shoulders.
