Overview

Organ Piercer's Briarspear is an epic two-handed polearm added in the Midnight expansion. The shaft is topped with a massive crown of interwoven thorns and barbed vines, resembling a crown of thorns impaled on a war spear. The dark wood of the haft and the greenish-brown growths reveal its origin in the Void-infested depths of the Voidstorm.
The weapon is intended for specializations that wield polearms: rogue and beast mastery hunters, monks, and druids in melee form. The silhouette is massive and predatory — this is not a ceremonial spear but a crude implement assembled from whatever grows in the dark.

The Organ Piercer's Briarspear can be obtained in several ways. For example, it drops from the Nullaeus Cache — a loot container left behind by Nullaeus, a formidable opponent of the first-season Midnight delves.
- Nullaeus ambushes you while you are clearing a delve: he appears suddenly along with a group of minions.
- When you deal damage to Nullaeus and bring him down to roughly half of his health pool, he teleports away and leaves the "Nullaeus Cache" on the ground.
- The weapon drops from this cache — along with other items from its loot table.
- The ambush can occur in seasonal delves located in the Voidstorm zone. The entrance to the Peak of Torment delve is located here:
- The item level depends on the delve's tier: the higher the tier you complete, the higher the stats of the dropped spear.
- The Nullaeus encounter is not tied to a weekly lockout for the item itself — you can farm the cache until the appearance you want drops.
To access the delves you will need a level 90 character and the Voidstorm zone unlocked through the Midnight story campaign.

Bring Nullaeus down exactly to the flee threshold: the goal of the encounter is to get the cache, not to drag out the fight with the minions.
The higher the delve tier, the higher the level of the dropped item — but only the appearance matters for transmog, so any tier will do.
Bring a supply of resurrections: progressing through high delve tiers requires a clean run without unnecessary deaths.
Check the cache's loot table after each opening — it drops several different items, and the appearance you need may not come on the first try.
If you play in a group, agree on priority for the polearm in advance so the spear goes to whoever needs the appearance.
