The sword drops in the Naxxramas raid — a flying necropolis above the Dragonblight in Northrend.

  • The source is not a boss but ordinary enemies: the Plague Beast that inhabits the raid's Plague Quarter.
  • The sword is bind-on-pickup and has a very low drop chance from this type of enemy.
  • Naxxramas is now long outdated and can be soloed on any class at current level — neither a group nor a specific specialization is required.
  • You can enter the raid once a week on the normal lockout, but the trash enemies are more interesting for farming the appearance than the bosses.
  • Since the drop comes from trash, it makes sense to clear the entire Plague Quarter rather than rushing straight to the bosses.

There is no guaranteed way to obtain the sword — it comes down to luck, so if you are unlucky, you will have to repeat the run in the following weeks.
