Stitchflesh's Scalpel

Game iconStitchflesh's Scalpel is a one-handed dagger added to World of Warcraft with the Shadowlands expansion (patch 9.0.2). Visually it is a short surgical blade: a narrow, straight blade of dull metal, a grip wrapped in leather straps, and a distinctive bony sheen that nods to the grim anatomical aesthetic of Maldraxxus. The weapon fits perfectly into "butcher" and necromancer looks.

The dagger is of rare (blue) quality and is intended for rogue specializations. For transmogrification this is irrelevant: once the appearance enters your collection, any class that uses daggers can apply it. The item itself binds when picked up, so the appearance unlocks only for the character who loots the weapon.

The Stitchflesh's Scalpel model is part of the Shadowlands dungeon dagger set. It has several color variants and one item that is completely identical in look, so you can collect the shade you need by different routes.

Stitchflesh's Scalpel drops from the boss Surgeon Stitchflesh — the third enemy in the The Necrotic Wake dungeon. This dungeon is located in Bastion, one of the zones of the Shadowlands.

  • The dungeon is available in three difficulties: Normal, Heroic, and Mythic, as well as in the Mythic+ format. The dagger's appearance is the same on any difficulty, so for transmog the fastest mode available to you will do.
  • Surgeon Stitchflesh fights on a raised operating table: his assistants throw a hook onto players, and the target must position so that the hook catches the boss himself and drags him down. While the boss is on the platform, he cannot be damaged — this is the only notable feature of the fight for those coming for loot.
  • The item's drop chance is low: the dagger shares the boss's loot table with other gear, so it does not show up on every single run. The dungeon can be repeated without weekly restrictions, so farming comes down to repeated runs.
  • At the current maximum level, the Shadowlands dungeon clears solo quickly and without risk. The boss has no mechanics that would block solo farming — the hook is simply ignored when you are noticeably stronger than the enemy.

The entrance to The Necrotic Wake is in the northern part of Bastion. The most convenient way is via Oribos: teleport to Bastion and head toward the dungeon entrance point.

Alternatives

The Stitchflesh's Scalpel model has one item with a completely identical look and several color variants from other Shadowlands dungeons. If the shade you need doesn't drop, you can obtain it elsewhere.

  • Suits a rogue for a pair of matching daggers — Stitchflesh's Scalpel and the look-identical Kris of Dark Temptation give a symmetrical combo with no visual mismatch.
  • Contrasts with the light cloth and bright armor of Bastion — a sharp but expressive choice for "fallen kyrian" looks.
  • Harmonizes with gear in swampy and gloomy tones thanks to its muted palette without shine or glow.

Enter The Necrotic Wake on Mythic difficulty via the dungeon finder or solo — the appearance does not depend on the mode, only the speed of the run matters.

If the dagger doesn't drop, don't fixate on one source: the same look is guaranteed by the Kris of Dark Temptation from The Maw.

Need a different shade of this model — check the Plaguefall, Sanguine Depths, or Mists of Tirna Scithe dungeons for color variants.

Check your appearance collection before farming: the shade you need may already be unlocked by another dagger from the set.

For fast farming, pick a class with high mobility and good single-target damage — the run to the third boss takes only a few minutes.
