Get Surgeon's Needle

Surgeon's Needle is an epic one-handed dagger added to World of Warcraft in patch 3.3 during the Wrath of the Lich King era. It is a main-hand caster weapon: the blade's primary stat is Intellect, so it is taken as a transmog appearance by priests, mages, warlocks, shamans and druids who want a compact dagger to match a healing or magical specialization.

Visually the blade is styled after the Scourge aesthetic of Icecrown Citadel: a dark edge serrated along the spine, dull steel with no trace of polish, and a hilt wrapped in coarse cord. The silhouette is short and predatory — this is truly a "needle" rather than a ceremonial blade. The in-game model carries the service name knife_1h_cotstrat_d_03, meaning this model series was originally prepared for the Stratholme dungeon, but Surgeon's Needle specifically debuted as a reward in the 5-player wing of Icecrown Citadel. The name directly references the sinister surgery of the Scourge — the needles with which plague masters stitch together the flesh of meat colossi.

Surgeon's Needle remains one of the few coherent caster daggers from Wrath dungeon content, so it is regularly farmed by transmog collectors assembling themed sets by expansion era. The appearance is especially convenient to obtain in the current Midnight season: Pit of Saron is in the Mythic+ rotation, and you can reach the boss you need through the standard dungeon finder without forming a separate group for the outdated heroic.

Surgeon's Needle drops from the first boss of Pit of Saron — Forgemaster Garfrost. He is a massive Scourge flesh colossus who oversees the smelting of saronite ore in the dungeon's forges.

• Dungeon: Pit of Saron — a 5-player dungeon from the Frozen Halls wing, opened alongside the Icecrown Citadel raid in patch 3.3.
• Boss: Forgemaster Garfrost, the first of the dungeon's three bosses.
• Difficulty: the dagger belongs to the heroic loot table. In the Midnight season the dungeon is available both through the heroic difficulty of the finder and through the Mythic+ system.
• Group composition: a standard five-man — tank, healer, three damage dealers. A coordinated group is not needed to farm the appearance.
• Attempt limit: there is no separate weekly lockout on the drop — you can run Garfrost as many times in a row as you like.
• Drop chance: roughly 10–11% per kill, according to Wowhead. It is one of the highest chances in his table, but still plan on about 10–15 repeat runs to collect the appearance.

How to get there

Pit of Saron is located in the Frozen Halls wing — the shared entrance hall of the three 5-player Icecrown Citadel dungeons: The Forge of Souls, Pit of Saron and Halls of Reflection.

• Zone: Icecrown, by the southern wall of Icecrown Citadel.
• Flight point: the nearest is the Argent Vanguard in Icecrown. You can also teleport to Dalaran and descend from there.
• Inside the Frozen Halls Pit of Saron has its own entrance portal; The Forge of Souls and Halls of Reflection are located nearby.

The fastest way to open the dungeon is through the Dungeon Finder tool: select Pit of Saron on heroic difficulty and you are placed straight inside, with no walk required. In the Midnight season Pit of Saron can also come up as a Mythic+ destination — if your key rolls into it, the dagger's appearance is still recorded in the Wardrobe the first time you receive the item.

The boss in brief

For farming the appearance Garfrost's tactics are not critical — at max level he dies very quickly. It is enough to know the general points:

• The boss periodically hurls a chunk of saronite at a random player — step aside so you do not get hit.
• Every 33% of health Garfrost retreats to the forges and reforges his weapon, after which he empowers his frost damage. At max level this is no longer dangerous.
• The tank holds the boss in the center of the hall while everyone else stands spread out — that way it is easier not to stack the area damage.

Surgeon's Needle has no confirmed color variants of the same appearance among modern transmog items — it is a standalone look from Wrath dungeon content. If the dagger refuses to drop for a while, it is worth looking at other dark caster daggers of a similar silhouette from the same era and picking a replacement for your outfit.
