The dagger drops in the The Underbog dungeon — one of the wings of the flooded naga complex in the marshy zone of Zangarmarsh. The dungeon entrance is located inside the central reservoir in the middle of the lake, with approximate coordinates of 54, 33.

The source of the loot is the dungeon's final boss, The Black Stalker. This is a massive creature from the depths of the swamp that completes a clear of The Underbog.
